This exploratory study proposes a conceptual framework for student resilience in longitudinal integrated clerkships (LICs), focusing on the first year of clinical training in a rural LIC. Through in-depth interviews with 19 medical students, professional staff, and clinician teachers, the study found that LIC students faced adversity due to challenges in the learning environment. However, their distress was mitigated by a secure, supportive learning environment, a profound learning journey, and the use of organizational structures. These inter-related themes form a conceptual model that suggests medical courses should go beyond providing tangible and emotional support, emphasizing the importance of agentic, reflective, and transformative learning for student wellbeing.
