The Millennium Development Goals aimed to reduce poverty and inequalities between countries, but often overlooked within-country disparities. The post-2015 Sustainable Development Goals (SDGs) emphasize reducing inequality both within and among countries, with Goal 10 specifically addressing this issue. Monitoring within-country inequalities requires disaggregated data to tailor effective policies. Many countries have made progress in health inequality monitoring, but more comprehensive data is needed, especially for noncommunicable diseases. Investments in various data sources and technical capacity are essential. Global standards for data collection and reporting will enhance international comparisons. The WHO Health Equity Monitor provides valuable data, and expanding such efforts will improve accountability and health outcomes. An equity-based approach, including universal health coverage, is crucial for achieving SDG health targets and improving population health. Collaboration among diverse partners is necessary to strengthen health inequality monitoring systems.
