The Two Georgias Initiative supported multisectoral community coalitions across 11 rural Georgia counties to build community capacity for health equity. A five-year, mixed-methods evaluation—including surveys, interviews, and document review—assessed outcomes across dimensions such as participation, leadership, skills, partnerships, resources, reflection, and power. Coalitions most consistently achieved resident engagement, leadership opportunities, skill-building around health equity, and strengthened partnerships, while resource mobilization varied. Many coalitions used data and community input to identify inequities, engage underserved groups, and deliver services directly, though fewer addressed structural root causes or shifts in power. Findings suggest community coalitions are effective for strengthening capacity, but greater impact may require centering health equity from the outset, deeper engagement of affected populations, technical and regional support, policy-focused action, and longer timeframes.
